Care Home Visits 

There are two Care Homes in our Practice area - Brooke House Care Home and Cresta Lodge Care Home.  We register patients residing at both Homes and the Care Home staff have registration forms that new residents and their families can complete.

All patients are offered a new resident health check on arrival at the Home and we visit the Homes routinely every Friday.

The Care Home Team are aware they can request attendance on those days we visit and they advise us of the need for the consultation.  Where there is a more pressing need, we visit on other days as required after contact from the Care Home Staff.

We are happy to update family members after the visit via the contact numbers we hold for next of kin.  We also welcome joint attendance with the resident and their family at our visits.
